1. Describe the interdisciplinary pain team structure at Providence Veterans Affairs Medical Center.

2. Explain the roles of a clinical pharmacy specialist managing chronic non-malignant pain in the outpatient setting.

3. Describe the health and economic consequences of chronic heavy alcohol consumption.

4. Identify factors associated with 30-day readmissions in veterans treated for acute alcohol intoxication and withdrawal management.

5. Discuss available interventions based upon identified factors to improve patient care and decrease 30-day readmissions.

6. Describe GLP-1 agonist mechanism of action in diabetes.

7. List the potential glycemic and non-glycemic benefits of using GLP-1 agonists.

REQUIREMENTS FOR CE CREDIT
A program evaluation must be completed at www.uripharmacycpd.org within 60 days of the live program date or of completing the home-study in order to receive CE credit. CE credit will be transferred to the CPE Monitor upon successful completion of the program evaluation. Transfer of CE credit should be verified at mycpemonitor.net where CE statements are available to print. Please refer to the evaluation instructions that follow for completing program evaluations and reporting CE credit.
